Recent guidance

  • Bisphosphonate Guideline Update and QI SearchesTo support physicians and the patients we serve, the North York Family Health Team is sharing a QI plan to identify and counsel eligible patients. This includes suggested discussion points when reviewing with patients as well as queries to identify eligible patients.

Tools for your practice

  • Geriatric Medicine Outreach Team - Get an in-home comprehensive assessment by an interdisciplinary team of Care of the Elderly physicians, a Nurse Practitioner (NP), Physical Therapist (PT) and Occupational Therapist (OT) to review issues related to mobility, cognition, general functioning, medications, home safety and more. Please note this is a one-time assessment with short-term follow up. 

  • North York E-Referral - Register to refer your patients quickly and easily to community specialists and services using OCEAN. 

  • SCOPE - Access an interdisciplinary health team for support through a single point of access. 

  • Cancer Screening - Find the latest tools and resources to help you optimize cancer screening. 

  • Telephone Advice Psychiatry (TAP) - Speak to a North York General Hospital psychiatrist without a patient visit about a clinical case by phone within days of request. 

  • Continuity of Care - A guide with updates relevant to your practice and downloadable letter templates to support you in communicating the appropriate scope of work to specialists. 

Health care promotion resources

  • Health 811 Resource Hub - Download free communications materials to promote awareness of the Health811 service in your practice and to your patients. 

  • Writing Sick Notes - To educate employers on changing their policies on sick notes for short-term illnesses, the Ontario College of Family Physicians (OCFP) recommends including this resource in your EMRs and clinic workflows. 

  • Toronto Community Crisis Service (TCCS) - Download a promotional flyer, poster and postcard to promote TCCS which offers free, confidential, 24/7 in-person mental health support from a mobile crisis team.
