Who we are
Our Primary Care Network (PCN) was established in April 2021 to represent the voice of primary care in North York. The Primary Care Advisory Council (PCAC) is composed of 11 primary care providers and a patient and caregiver health council member who represent the broader network of primary care providers in our decision-making. PCAC members sit at numerous committees to provide the primary care lens in NYTHP planning.
